Personal Plans Eligibility and Enrollment Guidelines
An individual is eligible to participate in GuideStone's Personal Plans if he or she meets both of the conditions listed below:
Eligible employees may enroll in a medical plan without providing evidence of good health if they experience one of the special enrollment events listed below. Employees must notify GuideStone® within 60 days of the date they experience the event. Coverage will be effective on the date of the special enrollment event.
New employment: New employees are eligible to enroll in a plan on their first day of work at an eligible employer.
Work schedule increases to 20 or more hours per week: New employees are eligible to enroll in a plan on their first day of work at an eligible employer.
Adding a dependent: Current employees who add a dependent* through marriage, birth or placement for adoption become eligible to enroll on the date the dependent was legally added.
Individuals who work for an employer who has 10 or fewer paid employees may apply for coverage by submitting the Evidence of Good Health Application at any time.
Please note: Employees of employers with more than 10 employees must experience one of the special enrollment events listed above in order to enroll. These employees may also be eligible to enroll in the employer’s group plan. Learn more about our Group Plans available to employers with at least two covered employees.
*Employees currently enrolled in medical plans may add dependents under the age of 19 at any time. In order for an employee to add a dependent older than 19, the employee must experience a special enrollment event or the dependent will need to provide evidence of good health if the employee is working for an employer with 10 or fewer paid employees.

Disability and Term Life Coverage
Eligible employees are required to submit the Evidence of Good Health Application for disability and term life coverage for themselves and all family members who are requesting term life coverage.
Employees approved for term life coverage are automatically eligible to add accidental death and dismemberment for the same amount as their term life coverage.
