Highmark BCBS

Download the BlueCard Global Core app or go to BCBSGlobalCore.com to help you find doctors, translate medical terms and access emergency care information when you’re outside the United States.

AllClear ID: (1-855-229-0079) Highmark BCBS provides AllClear ID to help members who are victims of identity theft. If you are enrolled in a Highmark BCBS health plan, you’re automatically enrolled in AllClear ID. For extra peace of mind, enroll in the AllClear ID credit monitoring service. Participants enrolled in credit monitoring are required to renew their enrollment annually at Highmark.AllClearID.com.

Vision Beneft: For individuals on most of GuideStone’s comprehensive plans, your vision benefit covers one annual eye exam per covered family member. The coverage does not include the cost for glasses or contact lenses. You must use an in-network provider to receive this benefit. The vision benefit is not available for GuideStone’s Value Health 5000, Secure Health 3000 or the Kentucky Baptist Convention’s Health Choice 3000K.

Cigna Dental

myCigna.com — This interactive website gives you ready access to account information, claims status, provider networks and other useful account maintenance tools. Dental health tips, a dental cost estimator, glossary of dental terminology, and dental health news and information are also available. Use the Cigna app to keep your dental information at your fingertips.

Cigna Dental PPO Network — Use providers in the Cigna Dental PPO network (Premier Dental Care and Choice Dental Care) to receive services at a discounted rate. This helps you have lower out-of-pocket expenses. Log on to MyCigna.com and use the provider directory appropriate for your plan to locate a dentist in your area.

Oral Health Integration Program (OHIP) — Enhanced benefits, including additional evaluations and preventive treatments, may be available for participants who are pregnant or have been diagnosed with one of the following health issues: cardiovascular disease, diabetes, stroke, head and neck cancer radiation, organ transplants or chronic kidney disease. Complete the Cigna Dental OHIP Registration Form to enroll in the program. Once enrolled, simply visit your regular dentist for treatment, and Cigna will automatically reimburse you for the amount of any co-insurance or co-pays resulting from OHIP charges.

Cigna Healthy Rewards® — Access discounts on a wide range of health and wellness products and programs. Benefits include weight management programs; massage therapy; acupuncture; vision discounts on contacts, glasses and frames; fitness club memberships and much more. These benefits are available to enrolled participants and enrolled dependents only. Not all programs and benefits are available in all states.

Term Life and Accident

Life Planning Financial & Legal Resources — This objective and personalized professional counseling service includes one-on-one financial counseling, a written personal financial plan and access to Ceridian-licensed counselors for your survivors in the event of your death or the diagnosis of a terminal illness.

Assist America — A 24-hour network of emergency medical and legal resources offering worldwide emergency assistance to active employees and their families who are traveling. Print your card today.

Accelerated Death Benefit — Allows terminally ill participants with a life expectancy of 12 months or less to receive up to 50% of the death benefit ($250,000 maximum) prior to death. Contact GuideStone for details.

Portability or Conversion of Coverage — You and your dependents can continue coverage by converting to a policy directly through Unum if you leave your employer or otherwise lose eligibility. Your employer can provide the forms you'll need to complete the conversion.

Add Children Without Underwriting — No underwriting is required to add a dependent child within 60 days of the child's birth, adoption or placement for adoption.

Education Benefit for Surviving Children — For qualified dependents, your GuideStone AD&D coverage includes an additional education benefit of 6% of the full amount of the AD&D benefit, up to $6,000 a year for up to four years. Your plan booklet provides requirements and details about this benefit.

Disability

Survivor Benefits — If you die after receiving disability benefits for 180 or more consecutive days, your survivor will receive a lump sum payment of three times your last month's gross disability benefit. Details are provided in your plan booklet.

Rehabilitation and Return to Work Program — To encourage individuals to return to work as soon as they become physically able, individuals receive an additional benefit for participating in a rehabilitation program. Details are provided in your plan booklet.

That’s why GuideStone® has teamed up with our long-term disability benefits provider, Unum, to offer a free employee assistance program. Through Unum's Work/Life balance services, employees will have access to live, one-on-one support, along with resources to help with family, health, life, money, work and legal issues.
