Important: The Care Basic Plan and Care Plus Plan pay healthcare costs based on Medicare-approved amounts. The participant pays all of the costs for services not covered by Medicare and costs above the Medicare-approved amount.

Important: If you sign up for Medicare prescription drug (Part D) coverage through another carrier, you may lose your GuideStone medical coverage.

  • Plans must be selected by the 10th of the month prior to your desired effective date.
