How to submit a paper claim

• Log on to Highmark's Web site.
• Select the “Your Spending” tab at the top of the page.
• Click on the “Spending Account” link in the middle of the page.
• Click on the “HSA — PNC” link at the bottom of the page. You will be redirected to the secure PNC Global Investment Servicing Web site.
• Click on the “Resource Center” link on the left-hand side of the Web page.
• Click on the Distribution Request form.
• Complete the form and send it to the address listed.
