How do I file a claim for term life benefits?
- Print a Notice of Claim (Life or Accidental Death and Dismemberment) or contact us at 1-888-98-GUIDE (1-888-984-8433) to request a claim form.
- Submit a copy of the death certificate with the claim form.
- If you are participating in the Group Plans, a representative will contact the employer for additional information.
Can I assign the term life insurance benefit to a funeral home?
If the beneficiary(ies) wants to assign the term life insurance benefit to a funeral home, a completed and signed assignment form must be submitted to GuideStone with the term life insurance claim. Each funeral home has its own form that must be signed by all beneficiaries. If no assignment form has been received with the term life insurance claim, the benefit check(s) will be made out to the beneficiary(ies).
How long does it take to process the claim?
Upon GuideStone's receipt of all completed and signed documents, it normally takes approximately 15 days to receive your benefit.
What happens if more than one beneficiary is listed?
The beneficiary designation form typically lists the percentages for each beneficiary. If no percentages are designated, the benefit will be divided equally between the beneficiaries listed.
