PPO Medical Claims Information

How do I file a claim if my provider has not filed it for me?

  • Submit itemized bills showing name of patient, diagnosis and services rendered.
  • If the claims are the result of an accident, include details about the accident on a separate sheet of paper — How? When? Where?
  • Complete the Highmark medical claim form or call GuideStone at 1-888-98-GUIDE (1-888-984-8433) to request one.
  • Retain copies of each claim filed.

Has my claim already been submitted?
If you have given your doctor permission to submit a claim, do not send it in yourself. Call your doctor to check the status of the claim he filed for you. You may also call Highmark Blue Cross Blue Shield at 1-866-472-0924.

What if my spouse has other medical coverage?
For charges incurred by your spouse, file first under the spouse's plan and retain extra copies of each bill. Submit a copy of the explanation of benefits from your spouse's plan to Highmark Blue Cross Blue Shield along with a copy of the corresponding itemized bill. If children are covered under both plans and the spouse's coverage is primary for the children, the same procedure applies.
