Special and Late Applicant Information

Contributory Plans
Employees may add medical and term life coverage for newly acquired dependent(s) without evidence of good health, as long as the change is reported within 60 days.

  • If the change is reported more than 60 days after the dependent’s original Eligibility Date, the newly acquired dependent(s) will be eligible for medical coverage during the Annual Re-enrollment Period for the following January 1. For Term Life coverage, if the change is reported more than 60 days beyond the dependent’s original Eligibility Date, evidence of good health must be submitted.
  • If the notification is not received within 60 days of the birth, a newborn may not be added to the participant’s medical coverage even if the participant has more than one dependent already on the medical coverage at the time of the child’s birth.

Non-Contributory Plans
Medical and term life coverage for newly acquired dependent(s) is automatic; however, the change must be reported to GuideStone. If notification of a change is delayed, a back charge to the Effective Date may appear on the monthly billing statement.

Note: In both Contributory and Non-Contributory Plans, eligible dependents can enroll in the dental plans at any time.

Late and special enrollees
Pre-existing Condition Limitation (PCL) period exists for 12 months. The PCL excludes coverage for a participant’s and his dependents’ conditions (whether physical or mental) for which medical advice, diagnosis, care or treatment was recommended or received within the six-month period prior to the coverage effective date for special/late enrollees. If the participant or his dependent(s) have pre-existing conditions, coverage for those conditions will be excluded under the health plan for 12 months.

Newborns and children adopted or placed for adoption have no PCL period if added within 31 days of the event.
