What is a deductible?

The up-front out-of-pocket expense. Participants must meet their deductible with eligible charges before claims will be paid.

What are Type I: Preventive services?

  • Routine oral examination— two per calendar year
  • Routine dental cleanings— two times per calendar year
  • Bitewing x-rays once every 12 months
  • Fluoride treatments for children under age 13—one treatment per 12 months

What are Type II: Basic services (restorative dentistry)?

  • Panoramic x-ray— once every 60 months
  • New fillings; replacement fillings— once every 24 months per filling
  • Simple extractions

What are Type III: Major services (crowns and major restoration)?

  • Endodontic treatment (root canal)
  • Dentures
  • Crowns

What are GuideStone’s waiting periods?

Based on your effective date of coverage, you may be required to satisfy a waiting period before certain services are eligible for benefits. 

  • Under the Premier Dental Care and Choice Dental Care Plans, there is a six-month waiting period for endodontic services, denture adjustments and, for children under 16 years of age, stainless steel or plastic crowns.
  • There is a 12-month waiting period for complex oral surgery and periodontic services and a 24-month waiting period for crowns, inlays, onlays, labial veneers and partial or full dentures.
  • There is a 12-month waiting period on orthodontic services under both plans.
  • The Guided Dental HMO Plan does not have waiting periods for covered services.

When will I receive ID cards for my dental plan?

Cigna Dental mails ID cards to plan participants within a month after becoming effective in the plan.  If you need to see a dentist prior to receiving your dental card, you may create an account on Cigna's website and print an ID card to use immediately (please allow at least a week for GuideStone to set up your account with Cigna).
