What is a health risk assessment?
A health risk assessment is a series of questions asked about your health and lifestyle. The assessment produces a report that identifies your health risks and suggests ways to improve your health. For best results, have this information handy – cholesterol, height, weight, blood pressure and waist measurements.
The assessment is designed to help you better communicate with your doctor. It equips you with questions to ask and a health assessment profile report for your doctor.

Where can I take an assessment?
- If you’re enrolled in a GuideStone health plan with Highmark Blue Cross Blue Shield as your network, use the new Wellness Profile online health assessment. Log into Highmark’s website and select “Challenges” under Programs in the bottom right-hand corner. Then, under Get Started, select “WebMD® Wellness Profile.”
- If you’re enrolled in a GuideStone health or dental plan with Cigna as your network, use this health assessment – "my health assessment." Visit Cigna’s website and log in. Select the "My Health" tab, then “my health assessment.”
- If your health or dental plan isn’t through GuideStone, you can use the Better Health Evaluator made available by WebMD and GE. Visit WebMD® and search for “Better Health Evaluator.”
